Photo ID and insurance card

Any relevant imaging (X-ray, MRI, CT) and reports

List of your current medications and allergies

Comfortable clothing that allows the provider to examine or treat the area involved

When You Arrive.

Check-in: We’re in Building 3, Suite 204 on the 2nd floor—plan a few extra minutes for parking and the elevator.

Privacy policy: For everyone’s comfort, video and audio recording are not permitted during your appointment.

Guest Wi-Fi: Available—ask our front desk for the current login.

If You’re Having an EMG/Nerve Conduction Study (NCS)

Day-of prep
  • Skip lotion on the limb(s) being tested.

  • Lower limb testing: Wear or bring shorts.

  • Upper limb testing: Wear a tank top or a loose-fitting shirt with sleeves that roll up to the shoulder.

After your test

Your provider will receive your results within one business day. Schedule your follow-up with your referring provider after your test date if you haven’t already.

Accessibility & Support

To ensure your safety and comfort during your procedure, we kindly ask that any patients who require assistance transferring out of their wheelchairs to the treatment table bring a capable support person to help with transfers. Our staff is unable to provide physical assistance for transfers. Thank you for understanding.

Billing, Insurance & Payments

Our staff verifies your benefits before your appointment and provides an estimated amount due for your procedure

A partial payment is due at the time of service (policy effective Sept 1, 2025).

Your payment can be applied to a prior balance, refunded, or left as a credit for your next injection—your choice.

Credit/debit (3% processing fee), HSA/FSA cards, cash, and checks.

Ask us about payment plans. If you’re unable to make the amount due prior to service, please give 24 hours’ notice to cancel or move your appointment to avoid the $25 cancellation fees.
