DME Evaluation & Prescription
For individuals requiring manual wheelchairs, power wheelchairs, walkers, hospital beds, orthotics, or other durable medical equipment (DME).
We provide evaluations to assist in determining which type of equipment and accessories will be most medically appropriate for your situation. After evaluation, we provide prescriptions to vendors who will further assist you in obtaining your prescribed DME.
Migraine Care
Chronic migraine care is typically treated through your Primary Care Provider. You will need to start there if just beginning to address your headaches. If, under the direction of your Primary Care Provider, you have tried two different preventative medications in two different drug classes and two different abortive medications in two different drug classes and not seen results, we may be able to assist you in finding a solution using Botox injections. (In essence, you will need to have tried four different drugs to curb the frequency and severity of your headaches without success in order for your insurance company to authorize coverage of Botox.) Secondly, an average of 15 migraines/month is required for qualification. If you meet these inclusion criteria and would like to see if Botox injections might work for you, we encourage you to schedule a Migraine consultation with our clinic.
Joint Injury & Functional Impairment Management
For sports, work-related or otherwise incurred joint injuries, we can provide ongoing care for all joints for patients of all ages. Conditions we see may include: osteoarthritis, tendonitis, joint sprains, and muscle strains.
We can perform diagnostic ultrasound in the office and perform ultrasound guided steroid injections, if warranted. We provide orthotic prescriptions to improve the use of the feet and/or hands for those struggling with full functionality due to neurologic impairments. We are well networked with the rehabilitation services in the area and if needed, will help you arrange for needed therapies. We can also provide medical referrals to an orthopedic surgeon if surgery could be warranted for your condition.
Nerve Conduction Studies/EMG Diagnostic Testing
We perform nerve and muscle function tests through Nerve Conduction Studies (NCS) and Electromyogram (EMG).
Nerve testing is for individuals concerned about potential nerve conditions such as: carpal tunnel syndrome, radiculopathy, or peripheral neuropathy. This test helps identify if a) there is a nerve problem, b) which nerve is involved, c) what part of that nerve has been affected, and d) the severity of the nerve dysfunction.
The EMG test involves a small needle with an electrode that allows us to hear and see how your muscles and nerves are working.
In some cases, diagnostic ultrasound is used to obtain images of certain peripheral nerves to help aid in the diagnosis of more serious nerve conditions.
Therapeutic Botox Injection Treatment
Spasticity is a neurologic condition that causes excessive muscle tightness resulting in impaired limb positioning and movement. Common conditions that cause muscle spasticity include Stroke, Cerebral Palsy, Traumatic Brain Injury, Spinal Cord Injury, and Multiple Sclerosis.
Botulinum toxins are muscle relaxers, designed to help decrease excessive tone in muscles caused by neurologic injury. We use botulinum toxins like Botox, Dysport, and Xeomin for treatment of muscle spasticity associated with the previously mentioned conditions, as well as cervical dystonia.
Cervical dystonia is a neurologic condition that causes neck tremors or abnormal neck postures.
Amputee Care & Prosthetic Prescription
We provide medical management for individuals with lower limb or upper limb amputations, as well as congenital limb loss.
We provide evaluations to understand what impairments you experience and to what degree. After evaluation, we will provide a prosthetic prescription to your Prosthetist. If you do not have a Prosthetist, we collaborate with a few local providers that we can refer you to. With the prescription from Dr. Shurtz, your Prosthetist will be able to contact your insurance company to assist you in understanding your options and coverage for manufacturing the most suitable prosthetic to help you improve your mobility and function.
Once a prosthetic limb is fitted, we provide collaborative care with your prosthetist, offering any adjustments or modifications needed to your prosthetic, over time.
We also provide assistance with various amputee-related issues, such as: residual limb skin breakdown/infections, excessive sweating, phantom limb pain, hip and back pain.
